Time-restricted eating improved the health of firefighters by reducing the risks of heart disease linked with shift work, according to a new study.

Working long shifts, up to 24 hours at a stretch, could be behind a number of health issues, including higher rates of diabetes and heart attacks.

But firefighters who stuck to a time-restricted eating planwherein the time of day determines when a person can eat, in this case 10 hours dailysaw those risks reduced.

Researchers say that their findings, published in the journal Cell Metabolism, could help others who work long hours, such as military personnel, nurses, transportation drivers, as well as new parents, whose schedules go awry when caring for a new baby.

Little previous research had been done to identify lifestyle interventions that could help prevent the health risks of working shifts.

But a new American study found that time-restricted eating (TRE) could be safely practiced in shift workers.

The research team also showed that TRE provided benefits to participants who had indications of cardiometabolic disease, a group of common but often preventable conditions including heart attack, stroke, diabetes, insulin resistance and non-alcoholic fatty liver disease.

Called the Healthy Heroes Study, the intervention focused on firefighters in San Diego, California.

Shift work is much more common than many people think, said co-corresponding author Professor Satchidananda Panda, of the Salk Institute.

Not only does shift work contribute to an increased burden of disease in our society, but it makes it hard for people with existing conditions like diabetes and cardiovascular disease to manage them.

Co-corresponding author Professor Pam Taub, a cardiologist at the University of California San Diego School of Medicine, said, Overall, firefighters are a pretty healthy group of people, but we found that for those who had underlying cardiometabolic risk factors like high blood pressure, high cholesterol, and hyperglycemia, there was some benefit to TRE, especially in terms of improvement in glucose levels and blood pressure.

Professors Panda and Taub have collaborated on research into TRE for several years. In January 2020, they published a study in Cell Metabolism that found that restricting the time of eating to 10 hours a day reduced body weight and improved blood pressure and cholesterol levels in people with metabolic syndrome.

In the current study, the team recruited San Diego firefighters, who work 24-hour shifts. There were 137 firefighters ultimately enrolled in the study; 70 followed TRE, eating all of their meals within a 10-hour time window, and 67 were in the control group.

All the participants were encouraged to follow a Mediterranean diet that was rich in fruits, vegetables, whole grains, and healthy fats. The subjects were followed for 12 weeks.

A barrier to conducting research studies with shift workers has been the subjects inability to attend the lab during regular business hours. The researchers got around that by going to the fire stations to apply wearable devices on the participants to collect their activity, sleep, and blood glucose levels.

They also customized an app that allowed the firefighters to log their food and sleep and answer study surveys; the app also enabled the researchers to send study materials and to guide the participants on following the recommended lifestyle.

The researchers found that the time-restricted eating pattern was both safe and feasible, as participants didnt report any problems with concentration, reaction times, or other issues and their quality of life generally improved.

Even those who were healthy with no underlying cardiometabolic risk factors had improvements in quality of life and in VLDL, which is a form of bad cholesterol, said Taub.

The research team wants to conduct similar studies with healthcare workers.

It is likely that the doctor will recommend measuring blood sugar in the morning at the time of Sehri, at noon, just before Iftar and 2 hours after Iftar. Self-monitoring of blood sugar not only helps avert the risk of hypoglycemia and hyperglycemia, but it also gives the doctor an understanding of whether the given medication and its dose are working, or if they need to be modified for optimal blood sugar control.

The daily calorie intake that the body requires needs to be balanced and evenly divided between Sehri and Iftar to avoid hypoglycemia and hyperglycemia. Foods such as whole grains, cereals, and beans have a low glycaemic index and release energy slowly. These can help to prevent sudden fluctuations in the blood sugar level. In fact, latest research, she says, has confirmed that those who consume one serving of whole fruit per day be they blueberries, grapes and apples have a lower risk of developing Type 2 diabetes. Another study suggests that if you already have Type 2 diabetes, eating whole fruits can help control your blood sugar.

But there are some ground rules. Those living with Type 2 diabetes can safely have one fruit every day. But the fruitshould be had on an empty stomach, not as an add-on to a meal or as an addition to other carbohydrates. This is the commonest mistake that many diabetics make. Have the fruit when you are at your hungriest, usually after you wake up. Also make sure that this single fruit becomes a part of your total carbohydrate allowance for the day. So, including the fruit would mean eliminating other calorie-rich foods to make way for it. Also, having a fruit after a full meal will increase blood sugar sharply as it would increase the total calorie load of that given meal, says Dr Gulati. If not in the morning, then a fruit can be had as a snack when hunger pangs kick in between large meals. Have it when you are really hungry.Mix the fruit with nuts, seeds and other proteins like yogurt for instance, she adds.

Of course, portion control matters too. Some fruits ought to be had as a lesser portion given theirglycaemic index (GI). As you know, the higher the score, the faster the absorption and the consequent rise in blood sugar. Usually, we say that one serving offruitshould contain 15 grams of carbohydrates and the total daily fruit intake should not exceed 30 gm. Otherwise, diabetics must know that low GIfruits(GI 20-49) include apples, avocados, cherries, guava, peaches, pears, and strawberries. Medium GIfruits(GI 50-69) include figs, grapes and oranges. Fully ripe bananas, mangoes and dates are examples of high GIfruits, says Dr Gulati.

But even when consuming low GI foods, we need to worry about the glycaemic load.Even one unit of glycaemic load approximates the effect of eating one gram of glucose. So dont gorge on low GI food. This is why the portion size becomes important. The culprit is always total calories, she explains.

As per the guidelines recommended by the American Diabetes Association (ADA), if a diabetic person is not allergic to any fruit then they may consume any fruit depending upon its glucose level and portion size. Being pregnant means forgoing some of your favorite food and beverages in order to keep both you and baby as safe and healthy as possible. This is especially true when it comes to anything caffeinated. While you dont have to avoid caffeine completely when youre pregnant, it is recommended to limit your intake. Most people are quick to cut out coffee from their diet, but that general rule of thumb applies to some teas also, like green tea. You might wonder if green tea is safe during pregnancy, or if its something you should wait to drink after you give birth.

Medical experts give green tea during pregnancy the green light, as long as its consumed in moderation. It is safe in limited quantities, says Sunny Soroosh, certified nurse midwife and nurse practitioner. Green tea might even have some benefits that are worth considering when you decide whether or not you want to have caffeine during pregnancy.

Green tea has a slew of health benefits for non-pregnant and pregnant people. High in antioxidants with a low sugar content, it can improve wellnesss, says Joy Cornthwaite, registered dietician, an OB-GYN nutritionist with UTHealth Houton and UT Physicians. Green tea contains natural chemicals called catechins, and studies have shown that the specific compound epigallocatechin gallate (EGCG) found in green tea can decrease inflammation and lower cholesterol, as well as promote heart health.

Green tea also contains an amino acid called L-theanine, and studies have shown that this can reduce stress and anxiety, which might be especially helpful during pregnancy. Some research has also shown that caffeine can improve brain function, mood, reaction time, and memory.

Green tea contains a decent amount of caffeine almost as much as espresso and some brewed coffees. According to the American College of Obstetricians and Gynecologists (ACOG), a moderate amount of caffeine consumption (defined as less than 200 milligrams per day) will not cause miscarriage or preterm birth, and an 8-ounce cup of green tea contains approximately 30-50 mL of caffeine.

The amount of caffeine in green tea can vary depending on type and steep time, so be sure to keep track of how much caffeine is in your green tea. You should also keep track of how much caffeine youve had from other sources, like coffee, chocolate, or other foods.

Most pregnant people can safely drink 200mg of caffeine daily during pregnancy, says Kelly Johnson-Arbor, MD, FACEP, FUHM, FACM, medical toxicologist, co-medical director, and interim executive director from the National Poison Center. However, because some people are more sensitive than others to the effects of caffeine, this recommendation may not be the same for everyone. Be sure to ask your OB-GYN how much caffeine you should have during your pregnancy.

While green tea has plenty of benefits, it also might have some risks. Green tea absorbs folic acid, which is a necessary nutrient in pregnancy because it helps to prevent neural tube defects such as spina bifida, Soroosh says. This nutrient is especially important in the first trimester, when many of the neurological structures are developing. She points out that, theoretically, green tea could prevent your body from absorbing enough folic acid and could cause neural tube defects (something that this 2012 research found), but you would have to drink a lot of it for that to happen. This is one reason people may avoid green tea completely or at least during the first trimester.

There are also some risks associated with having too much caffeine during pregnancy. In 2021, the National Institute of Health reported that pregnant women who have even as little as half a cup of coffee a day on average gave birth to slightly smaller babies than pregnant women who did not consume caffeine. Small birth size can put infants at a higher risk for conditions like obesity, heart disease, and diabetes later in life.

Some studies have also shown that consuming high amounts of caffeine during pregnancy can be associated with negative fetal outcomes, including miscarriage. Caffeine can cross the placenta, but the fetus cannot metabolize it, Johnson-Arbor says. In addition, the bodys metabolism of caffeine is slower in pregnant people compared with non-pregnant individuals, resulting in increased fetal exposure to the drug.

Lastly, Cornthwaite notes that some studies have found some correlation between an increased risk of dehydration, undesirable changes in blood pressure, low birth weight, and miscarriage in pregnant women who drink more than 200mg of caffeine daily.

At the end of the day, green tea is considered safe for pregnant people to drink, as long as they have it in moderation (less than 200mg of caffeine each day). That said, it does come with some risks that are worth considering, and its always something to speak with your OB-GYN about. Whether or not you drink green tea during pregnancy is a personal decision.
