People who have had cancer may in some cases have a higher than average risk of developing a new cancer that is, a cancer not related to the spread, or metastasis, of the original tumor. While the vast majority of people diagnosed with cancer do not develop a different malignancy later in life, its important for survivors to be aware of factors that can put them at risk.

Pinning down the extent of that risk is difficult. For one, the rates of many cancers increase with age: the older a person gets, the more likely he or she is to develop certain forms of cancer, regardless of whether he or she has had cancer in the past.

People whose initial cancer was associated with certain environmental factors or lifestyle issues such as exposure to harmful substances at work or at home, smoking, excessive sun exposure, poor diet, or drug abuse may be at risk for future cancers of the same or different type. In many cases, such risks can be reduced by adopting healthier habits or reducing or eliminating exposure to substances that triggered the first cancer. Examples include quitting smoking and avoiding sunburn.

Like age, there are several factors beyond an individuals control that can influence the risk of a second cancer. One of these is genetics. People who inherit genetic mutations or other abnormalities can be at risk for a variety of different types of cancer. Individuals with a mutation in the BRCA1 or BRCA2 genes, for example, have a heightened risk of cancers of the ovaries and fallopian tubes in women, breast, prostate (in men), and pancreas. Such individuals remain at elevated risk for these cancers even if theyve undergone treatment for one of them.

In some cases, patients can take steps to reduce their chances of developing a second cancer. A woman with a BRCA gene mutation who develops breast cancer may have the option of having her breasts and ovaries surgically removed to lower her risk of future breast or ovarian cancer.

Some of the therapies used to treat cancer can also raise the risk of other malignancies. Radiation therapy and many types of chemotherapy work by damaging DNA in tumor cells, causing them to die. But such damage can occur in normal cells as well, potentially putting them on a path toward cancer.

As physicians have gained experience with different types of therapy, theyve often been able to develop reduced-intensity regimens that effectively kill cancer cells but produce fewer long-term risks. This is particularly true in the field of childhood cancer.

For many survivors, especially those whose treatment or genetics may place them at higher risk of a second cancer, physicians may suggest more frequent screenings to detect any such cancers at the earliest, most treatable stages. Patients should talk with their physician about which strategy makes the most sense for them.

The mostly harmless new Netflix series The Goop Lab makes it easy to forget the damage Gwyneth Paltrows pseudoscience-y brand of "wellness" can do.

Youve gotta give it to Gwyneth Paltrow: Shes no dummy. The promotional photo that looks like shes trapped inside a many-layered vulva; the vagina-scented candle that costs $75 (sold out, good grief); her surprisingly delightful role on The Politician; her cool-mom presence on Instagram, leaving a faint breadcrumb trail of proof that actually she has a great sense of humor it all creates the image of Paltrow as someone far more self-aware than her wellness brand Goop, on its face, would suggest.

It remains kind of a bummer that someone as interesting as Paltrow is using her charisma to recommend and sell a variety of potentially risky pseudoscience treatments and products to her large, enthusiastic audience. But based on her newest project, a Netflix series called The Goop Lab that debuts January 24, it looks like this is the Paltrow were getting from now on.

Each episode of the show explores one alternative health or wellness trend, following an expert or enthusiast whos offering a different way to live a better, happier, healthier life. Paltrow dips in and out, usually interviewing the leading expert and sometimes acting as the guinea pig herself. (She does not, regrettably, do mushrooms or masturbate on camera.) And each Goop Lab episode begins with a disclaimer: The following series is designed to entertain and inform not provide medical advice.

Maybe the Goop team learned something from the lawsuit it settled in 2018, over some deeply unscientific claims about the benefits of inserting a jade egg into your vagina. At the time, Goop suggested that the egg could help balance your hormones (it is a rock shaped like an egg) and help prevent uterine prolapse (it is still a rock shaped like an egg). The site's editors have since removed the offending language from the product description.

Paltrow and her staff have built a business on pseudoscience that targets women where our anxieties are.

A pretty sizable chunk of Goops website is dedicated to convincing you to buy luxury goods you absolutely do not need: a pair of low-rise jeans that only go up to size 31 ($295), an infrared sauna blanket ($500), or a pair of made-to-order pearl drop earrings ($16,780). Most of these things are unnecessary, but perfectly harmless. Would you like a $200 dopp kit that only holds maybe six products? Knock yourself out, kid. But clicking on the wellness section of the Goop site opens the door to a much more nefarious part of the business. Theres a wide range of (highly suspect) information on fasting-mimicking meal plans (briefly mentioned on Goop Lab), a summer detox guide, and an endless selection of supplements that cost as much as $90 including something called High School Genes, a one-month supply of pills formulated for women who feel like their metabolism might be slowing down.

So youd be forgiven for walking into The Goop Lab with derision, prepared to be sold some (possibly literal) snake oil or maybe a CBD patch that you put on your butt to calm your detox teainduced diarrhea or perhaps a $42,500 pillow made entirely of rose quartz. (Its very uncomfortable but very exclusive.) It may be either a relief or a disappointment to learn that The Goop Lab feels like it comes from a different, much tamer (and less product-led) point of view than the Goop brand overall. And thats actually the biggest problem with The Goop Lab: There really isnt one.

The recommendations dispensed are largely reasonable, if a bit unorthodox when compared to traditional medicine microdosing mushrooms to help with PTSD, psychic readings, looking at your own vagina, jumping into cold water to keep yourself feeling young. Any science The Goop Lab presents is generally flimsy, but its really not about the science. The various slightly woo-woo topics explored have been covered many times before, sometimes by this very media company.

The trouble comes when you compare the series with Goops broader branding and strategy. Paltrow and her staff have built a business on pseudoscience that targets women where our anxieties are: Am I getting enough sleep? Am I attractive? Am I thin? Am I sexually fulfilled? Hows my skin? Hows my hair? Hows my overall sense of self?

Often, were not happy with the answers to those questions which leads people to buy products that dont work or take advice thats downright dangerous. The Goop Lab, meanwhile, is much more innocent and far harder to hate than the e-commerce empire that spawned it. So it acts as a form of brand rehab for Goop and Paltrow in general but theres no indication that the underlying Goop ethos has actually changed.

Some episodes of The Goop Lab may actually be emotionally impactful if youre a cis woman struggling with your body. The Pleasure Is Ours focuses on sexual health and shows more vaginas than youd even see in an eighth-grade health class. The episode follows an accountant who works for Goop, a queer woman originally from Shanghai, struggling to open up intimately with a partner a rare moment of the Goop brand not being aggressively white, rich, and capitalist. Sex educator (and absolutely delightful, brassy broad) Betty Dodson recommends some pelvic floor exercises, which can have benefits for many women, but theres no one urging viewers to buy any mysterious sprays or unnecessary wands or douches. (The famed egg, alas, does not make an appearance.)

Other episodes are less effective. Cold Comfort follows Wim Hof, an extreme athlete who believes that his particular breathing method can help the body withstand subzero temperatures, depression, stress, and, most worryingly, bacterial infections. The idea that if you just believe in yourself and breathe like Hof, you, too, will be able to fight off hypothermia and E. coli is both patently ridiculous and outright dangerous. I dont know who needs to hear this, but: Breathing exercises will not save you from freezing to literal death.

Believing in the otherwise unbelievable is easy to do if youre willing to take a small leap of faith.

In another episode, Paltrow puts herself through a fasting-mimicking diet for five days, which effectively allows her little more than a watery, miserable soup, a bunch of tea, and a nut bar each day. What Goop frames as wellness is also often focused on weight loss and food restriction, even if no one says so directly. The Health-Span Plan episode is purportedly about your biological age, but it also advocates for incredibly expensive facials, diets that punish the body and the spirit, and a general focus on clinging to physical youth rather than prioritizing something like, say, your general quality of life.

Much of The Goop Lab is just plain weird and barely worth picking apart. What are you supposed to pull from the episode where Julianne Hough joins Paltrow and her Goop employees (who are featured in all the episodes, seemingly of their own free will, which Im sure their HR department just loved) as they get their energies realigned? The episode is 29 minutes of people writhing and grunting on tables with no scientific proof or explanation for whats apparently happening to them. The expert leading the episode John Amaral, a go-to energy healer for the rich and famous seems to believe that changing your bodys energy (which is basically just having some guy wave his hands over your body until you convulse, I think?) can change your rate of cell growth which, honestly, isnt the dumbest thing Goop has ever suggested.

Believing in the otherwise unbelievable is easy to do if youre willing to take a small leap of faith. If youre watching The Goop Lab to begin with, youre likely already prepared to jump. But the show is enough of a departure from the absurdity of Goops website, and so much gentler in its approach to overpriced, overhyped, unnecessary wellness trends, that youd be forgiven for forgetting that Goops fundamental business model is still profiting off gullible and vulnerable consumers who buy into the dubious promises it makes. With The Goop Lab, it seems like Paltrow is making a play to bring her brand out of the jade egg era into a new phase of (relative) credibility. But of course, the egg is still for sale, for $66 if it ever comes back in stock.

How does salt raise blood pressure?

Additionally, if youre taking diuretic medication to manage high blood pressure (which forces the kidneys to remove more fluid from the bloodstream), too much salt counteracts this effect, reducing how effective the medication is.

For two, persistent extra strain on artery walls causes them to become thicker (to cope with the extra force of blood) and this makes the space inside the arteries smaller and raises blood pressure even higher.

Eventually, this could lead to arteries bursting or becoming so narrow that they then clog up, and any organ the arteries were delivering blood to become starved of oxygen and nutrients.

To illustrate, clogged and damaged arteries prevent enough blood flowing to the heart or brain, causing a heart attack or stroke.

Clearly, its a great idea to reduce the amount of salt consumed in your diet if you want a longer, healthier life.

There are a host of other foods particularly high in salt that are best to avoid (or find low-salt alternatives to). These include:

Food manufacturers now display how much salt content an item of food contains in a traffic light colour code system.

Green means the salt intake is way within the maximum recommended daily allowance.

Amber is a cautionary colour, symbolising the food item should be consumed only now and again.

And red should be considered a one-off treat, as the proportion of salt is very bad for your health.

A report published in The Lancet has claimed that one in five deaths globally can trace its roots to poor nutrition. As stated by the report, we are living in a time when the poor diet is a leading cause of thousands of deaths in India annually. Could the Eat Right Movement be a means of combating this?

Indeed, malnutrition was responsible for almost seventy percent of child deaths in India in 2017. Such a figure places malnutrition firmly into the top spot as Indias most prominent cause of infant mortality.

Conditions caused by malnutrition affect significant numbers of children. According to recent figures by the Comprehensive National Nutrition Survey (CNNS), stunting affected 35 percent of children under five in India and wasting affected seventeen percent. 33 percent of children are underweight, including 35 percent of children aged five to nine years; ten percent being severely underweight.

In response to the rise in deaths attributed to malnutrition as well as a major shift in death toll from infectious disease to noncommunicable diseases (NCDs) the Government of India launched the Eat Right Movement. As reported by Health Issues India at the time, the movement was influenced by the India State-level Disease Burden report, which noted dietary risks, which include diets low in fruit, vegetables and whole grains, but high in salt and fat, were Indias third leading risk factor for health loss in 2016.

The Eat Right Movement aims to utilise social media and mass media platforms such as television and radio, as well as using information pamphlets in order to effectively target both the urban and rural population. The information delivered is aiming to bring awareness of the importance of a healthy diet and the risks a diet high in trans fat and salt can bring.

India currently shares a complicated relationship with food. A dual burden of malnutrition has arisen, with both undernourishment and obesity presenting issues. Though obesity is often considered exclusive of malnutrition, this is a misconception. A person may be malnourished through being severely underweight, or they may be overweight through excess calorie consumption and still be considered malnourished due to various nutrient deficiencies arising from the consumption of unhealthy, processed food while foregoing fresh produce such as fruit and vegetables.

The Eat Right Movement aims to tackle both of these issues. This, combined with exercise programmes such as the Fit India movement, could have a major impact on health within India. NCDs are Indias predominant causes of death. Malnutrition takes the most prominent position as Indias most common cause of death in children. Assuring diets across the nation are healthy could eliminate a major risk factor of NCDs, as well as notably improving the health of children with the potential to reduce child mortality by a considerable margin.

Your Teeths Support Network

Your teeth have a demanding job. They chew your food; help you speak and support your lips and face. To work effectively, your teeth must sit firmly in your gums but also move slightly under pressure. This is where your periodontium comes in. This part of the mouth includes the cementum, the periodontal ligaments, the gums and the alveolar bone. Cementum surrounds the tooth roots, periodontal ligaments link the cementum to the alveolar bone and gums cover the bone. Together, these structures hold the teeth securely but not too rigidly.

Gum Disease

However, if bacteria and plaque invade your gum tissues they can bleed and become red, sore and swollen. This first stage of gum disease is called gingivitis.

As the other structures break down, the pockets deepen, and the teeth can fall out. Periodontal disease is associated with illnesses that affect the whole body, includingdiabetesandheart disease.

What Activities Damage the Periodontium?

To reduce your chances of developing gum disease, it is recommended that you avoid smoking and eating a diet rich in fruit and vegetables. Smoking more than one-and-a-half packs of cigarettes a day increases the risk of periodontal disease by a factor of six and smoking even one-half of a pack of cigarettes per day increases the disease risk threefold. Smokers are also more likely to develop hard, calcified deposits at the gumline called tartar, and they may suffer from greater bone loss at the tooth roots.

There is evidence that your dietary choices may also help prevent gum disease. Eat a healthy diet that includes nuts, legumes, fruit, vegetables, vegetable oils and fatty fish to help to prevent inflammation that may affect the gums.

Healthy Gums

Keep up with a good daily oral hygiene routine and attend regular dental visits tokeep your gums healthy. Use a soft-bristled toothbrush to gently brush your teeth twice daily, and brush at a 45-degree angle toward the gumline. Floss once per day, making a C-shape with the floss around each tooth to remove food debris and plaque. Rinse once per day with a mouthwash that protects gum health. Finally, visit your dentist at least twice a year for a checkup. They can spot signs of gum disease that you may have missed and treat them before the condition becomes serious.

If you keep your gums clean and healthy, youre protecting the rest of your periodontium. Ask your dentists and hygienists advice about how to keep your gums in good condition. Youll be on your way to reducing your risk of serious gum disease and tooth loss.

Nearly 50% of the entire study pool drank some form of milk every single day and a quarter consumed dairy on a weekly basis. Of these, a little more than 30% drank whole milk, exactly 30% drank 2%, 17% drank skim, 13% preferred non-dairy alternatives like soy or almond and a modest 10% regularly drank 1% milk.

Believe it or not, over the years, the average dinner plate has increased in size, and its no wonder that the same can be said for the average persons weight.

When eating, people tend to finish off all the food on their plates. This can contribute to increased weight gain. Yet, eating off of a small plate may trick your brain into thinking that youre eating more than you are. For instance, a study published in the International Journal of Obesity found that a serving on a bigger plate can make said serving look smaller, causing one to add more food to the plate.

Protein is used by the body for cell growth and metabolism. However, it can also be used to help you lose weight. This is because protein has been found to regulate the appetite hormones GLP-1 and ghrelin. This then helps increase feelings of fullness as well as reduce hunger.

In fact, research published in the American Journal of Clinical Nutrition found that increasing protein intake by 15% resulted in participants eating 441 fewer calories per day when then led to an 11-pound weight loss over a 12-week period all without any intentional food restriction.

Increase your intake of protein by eating more chia seeds, hemp seeds, legumes, nuts, lean meat, fish and quinoa.

According to research published in the Journal of the American College of Nutrition, a form of fiber known as viscous fiber has been found to effectively help with weight loss by increasing fullness and reducing food intake.

If youre looking to increase your intake of viscous fiber, look to Brussels sprouts, asparagus, flax seeds, oats, and oranges.

If you really want to lose weight in 2020, you may need to reevaluate where you keep your unhealthy snacks, if at all. First prize is you banish them from your home. Replace them with nuts. fruit and veggies. But if this is a leap too far, and you need them for entertaining guests, then store your cookies and cakes out of sight to reduce temptation.

Research published in the journal Health Education & Behavior found that keeping high-calorie and unhealthy foods in your line of sight can increase hunger and cravings, causing you to eat more and increase your weight gain.

That said, its preferable to store unhealthy foods out of sight, perhaps in your cupboards. Also, keep healthy foods such as fruits, visible on your countertops and front and center in your fridge.

Its a new year and with that can come new stressors, and that can soon lead to increased weight gain. This is because research has found that increased stress levels means increased levels of the hormone glucocorticoids, which has been found to increase ones appetite (3).

Great ways to better manage your stress in 2020 include yoga, meditation,breathing exercises, as well as spending time outdoors.

It may be common to binge-watch while eating your favorite meal. Yet, this could be the biggest hindrance to your weight loss goals. Additionally, its not just watching TV.

According to research published in The American Journal of Clinical Nutrition, any distractions during eating can cause one to eat about 10% more than usual. Therefore, moving forward, its important to focus and practice mindfulness when eating. So if you want to lose weight, then you must do your best to keep your phone away from the dinner table and keep the TV off.

Intermittent fasting is an effective way to achieve weight loss. Instead of running on fuel from the food you just eat, fasting allows your body to tap into reserves. For example, fat, which accumulates on the body, can be burned whenever food supply grows scarce. This results in a slow, steady weight loss that can be a huge benefit.

Since fasting is often incorporated as a lifestyle change instead of a temporary fix. This type of diet is much more sustainable than many other crash diets. All cannabidiol (CBD) is not created equal in clinical practice in terms of implementation. The most frustrating thing for clinicians and also for patients is when an intended protocol doesnt work. As I discuss later under the With treatment, start low section of this article, there are ways to use CBD most effectively.

You probably werent taught this in medical school, but CBD works on the endocannabinoid system (ECS). The ECS contains CB1 and CB2 receptors, in addition to other receptors that are activated by the naturally occurring endocannabinoids made in our bodies, called anandamide and 2-AG. We all have CB1 or CB2 receptors within our body if you have a cat or dog, they have these receptors too. All mammals do. How could it be that we did not even know of the predominance of the ECS until 1992?

Patients coming in worn out can be a sign of endocannabinoid deficiency and the inability to support their pathways optimally, much like adrenal fatigue. Phytocannabinoids like CBD obtained from hemp products can act on the same receptors as endocannabinoids made in the body. CBD receptors, much like opioid receptors, control pain, but via a different mechanism.

Patients arrive at our clinical practices fatigued, anxious, in pain, inflamed, and lots reach a point of adrenal burnout, adrenal fatigue. Indeed, that is often the case because they are suffering from an endocannabinoid deficiency. We see people who have low adrenal function at noon, and in the evening, they are tired, dragging, eat lunch, get a little spike, and then they start free falling again. Theyre flat-lining. How is it that we wake up each morning anticipating with trepidation and run with go, go, go pace and finish the day glad to have survived another day? Life is meant for more than that; it is time to shift our goals from merely surviving to thriving mode!

Researchers observed approximately six years ago that people were more anxious and depressed in the middle of the summer than ever before, a new phenomenon. This goes against what we previously believed, that depression spiked mostly in the holiday season and Thanksgiving and after the winter solstice. But now were seeing people are worn out even in summer. Indeed, the endocannabinoid system plays a role here.

Anandamide is one of our endocannabinoids. When it becomes lower, patients are more anxious. There is less healing of the brain, less neurogenesis, and the hypothalamic-pituitary axis is triggered, causing a whole cascade of events to occur. What we now know is that the endocannabinoid system naturally, or with the use of CBD, alters our microbiome in our gut, beneficially affecting the gut-brain axis. The GI tract is the site of many receptors. We make about 70% of our serotonin, the happy-brain chemical, in the GI tract. About 70-80% of our immune system is in our GI tract, so is it any surprise that we have CB1, CB2 regulatory pathways in our GI tract that work via the endocannabinoid system to control inflammation? These same pathways can be regulated using an innovative CBD strategy.

With endocannabinoid deficiency symptoms patients can get migraines. We thought migraines were just triggered by tyramine-rich foods, such as cheese, fermented foods, and salami and other such foods; along with other triggers as well.

However, the endocannabinoid pathway is designed to control pain and inflammation and also regulate the immune system to a large degree. I have found in my clinical practice that innovative CBD strategies can be useful in supporting the health of migraine sufferers.

The body is designed with innate intelligence to sustain optimal performance. People ask, Well, how is CBD metabolized? Its primarily metabolized through the liver, and as a result, it goes through the cytochrome 450 and other pathways.Wellness is ultimately about homeostasis. We all know the term entropy, the tendency for the universe to move toward chaos. But our body exerts a phenomenal amount of energy to maintain order and wellness. However, if the endocannabinoid system becomes deficient, this will adversely affect wellness. This endocannabinoid deficiency, in turn, affects the microbiome, contributing to a less healthy endocannabinoidome.

CBD can conserve anandamide that is made by the body, and CBD can support anandamide conservation in an attempt to bring the body back into homeostasis. The endocannabinoid system supports brain health. By treating the brain, it addresses the pain, because if we didnt have a brain, we wouldnt feel the pain from a perception perspective.

Anxiety also is a biochemical process. Its real. A lot of people say, Its all in my head. No, stress is real because its a physiological effect. There is biochemistry involved in anxiety. We want to use innovative CBD strategies to support calming pathways and bring peace and harmony into the brain.

If a patient has brain inflammation, neuroplasticity is not going to be as healthy or robust. And the fact is, on a typical day, we lose about 86,400 brain cells. CBD is an anti-inflammatory application. And from a neuropathic perspective, activation of cannabinoid receptors is essential. Ive looked at research involving the endocannabinoid system relative to cancer medications and so forth. Not being an oncologist, I am not suggesting this is a treatment for cancer, but there are pain, stress and immune challenges that often need to be associated with this disease state, so visiting with ones provider in this arena is vital.

We can start manifesting symptoms of migraines, fatigue, irritable bowel, whatever it might be, as a result of being subpar, insufficient or deficient of endocannabinoids. An endocannabinoid deficiency manifests itself as simply that the body has gotten worn down, much like low adrenal function, often called adrenal fatigue. So we need to support that pathway to bring it back on track like we would any other functional biochemistry, which our body is designed to do. And since were designed to make endocannabinoids, we need to bring those levels back up to par.

The goal is to start low and go slow, as a little bit can go a long way for many patients. More is not always better, as each person responds differently based on genetics, biochemistry and individual need.

Just like if youre performing an adjustment, a little bit of a movement might get the job done. Work with gentle modulation of the endocannabinoid system. Youre working within the innate ability of the body.

Fifteen to 25 milligrams per day is what I start my patients on. In a week or two, if I need to move them up to 25 mg two to three times a day, I will. Once again, if a little bit gets the job done, why more? It is of paramount importance to respect the innate delicate balance physiology of the body. Some patients will need much higher doses because of their endocannabinoid insufficiency or potential genetic polymorphisms. Youre going to encounter genetic polymorphisms, or what I termed mutations of the CBD receptors and the CBD pathway.

I never go to high-milligram doses, especially on patients taking pharmaceutical drugs, without working with their pharmacists, because CBD can affect similar liver detoxification, as well as other biochemical pathways of how drugs are metabolized, much like grapefruit juice, can impact many of the same medications. We want to also support those drug-metabolizing pathways with diet and lifestyle.

CBD, when dosed properly, can positively supplement a health-promoting diet and lifestyle. Its not a substitute for these things. So, we want to ensure were taking steps to support the body at a foundational level. Additionally, we always want to ensure the THC levels in the CBD you are using are less than 0.3%, or that the CBD contains no THC, when targeting hemp-derived CBD.

Endocannabinoid deficiency is associated with low cortisol, stress and adrenal fatigue. Were all getting older. Our mitochondria are wearing down. Our nitric oxide levels arent as robust as they used to be, and we need support at a fundamental level. In regards to the 65 potential molecular targets that are currently known for CBD, there is a lot of untapped potential here relative to stress and aging. Innovative CBD use is a new area; it can be a scary area, but remember, our bodies have been producing endocannabinoids for millennia. All were doing is supporting a natural pathway.

MONTE CARLO Outside an office building not far from where Novak Djokovic resides, a Lamborghini was parked on a street lined with luxury hotels, restaurants and sports cars.

Inside, Novak Djokovic was talking about hard times.

Ten Deutsche marks, I remember 10 Deutsche marks, said Djokovic, the 16-time Grand Slam singles title winner who went into the Australian Open this week as the reigning champion.

As he spoke, he slammed his hand on a conference room table, just as his father, Srdjan, had once slammed a 10-mark bill on the kitchen table of their cramped rented apartment in Belgrade.

This was during the violent breakup of the former Yugoslavia in the 1990s. Djokovic does not remember precisely when, but he remembers his fathers words.

Ten Deutsche marks was like 10 dollars, and my father said, This is all we have, Djokovic said. And he said that more than ever we have to stick together and go through this together and figure out the way. That was a very powerful and very impactful moment in my growth, my life, all of our lives.

By 1999, Djokovic and his family had found a way for him to leave Serbia at age 12 for Niki Pilics tennis academy in Munich, the start of his journey toward the top of his sport.

Djokovic has shown remarkable resilience and drive, spurred by the memories of growing up in the midst of conflict, privation and uncertainty.

But at age 32, after earning more than $100 million in prize money and hundreds of millions more from sponsorships, he is in a very different time and place.

He explained that he and his wife, Jelena, and their children a 5-year-old son, Stefan, and a 2-year-old daughter, Tara start their mornings in Monaco by greeting the day on the balcony of their apartment, which overlooks the Mediterranean Sea.

We wake up early, because I take my son to school, so we prepare our juices in the morning in the kitchen, and then we go out and watch the sunrise, and then we do our hugging session and singing session, he said. And we do a little yoga.

Djokovic chuckled, a bit self-consciously. But he believes the private ritual reflects how he has changed. In his view, he is no longer playing tennis to prove himself but to improve himself and the lives of those around him.

For the first 15 minutes of an interview last month, there was no need (or opportunity) to ask a question. Djokovic, who rarely gives in-depth interviews about his personal life but speaks in long, sometimes meandering paragraphs, shook hands firmly, took a seat and started riffing, apologizing intermittently for the monologue.

Everybody talks about trophies, achievements, records, history, and Im really blessed to be in a position to be one of the guys in the mix and in the midst of these kinds of conversations, he said. I am really grateful for the career Ive had, but for me right now, tennis is more a platform than an obsession about individual achievement.

And yet the tennis world is perhaps more focused than ever on individual achievement as Djokovic, Roger Federer and Rafael Nadal jostle for history and championships. Collectively, they have dominated mens tennis for a decade.

Nadal has won 19 Grand Slam singles titles, just behind Federers 20, the record for men. Djokovics 16 rank him third, and he has a good chance for more if he continues to move, compete and serve as well as he has recently. He is nearly one year younger than Nadal, and six years younger than Federer.

Djokovic, like Nadal, is playing down the title race even if, unlike Nadal, he has made it clear that Federers career record is an objective. He recognizes that the numbers and his staying power present an opportunity.

I dont see tennis anymore only as Im going to go there, and Im going to win the trophy, do everything possible to achieve that, and once thats done its done, and thats the only reason Im playing, he said. Ive finished with that kind of chapter in my life. I guess through the evolution of my life I came to the stage where its more than that.

He and Jelena want to grow their family foundation, which is focused on early childhood education. They want to finish a book on wellness and help answer the questions Djokovic says he gets about how to live a purposeful, healthy life.

Djokovic at this stage is speaking similarly to another champion when he was in his early 30s: Andre Agassi, the once-tormented American who found new energy for tennis after founding a school in Las Vegas, his home city.

Djokovic was never as conflicted about tennis as Agassi. But Agassi, now 49, helped coach Djokovic during one of the most difficult periods of his career 2017 and early 2018 before an elbow operation finally helped him shake free of a funk. Agassi, who never had a formal contract with Djokovic, has said he thought his friend was too slow to have surgery. But the two remain close, and Djokovic said he sees parallels in their lives and careers.

He was one of the very influential people in my life, who actually allowed me to come to all of these realizations even more, Djokovic said. He added: If you see things from a larger perspective, its quite interestingly the same in terms of understanding what the next step in life is. I will eternally be grateful to him.

Two of the current Big Three are relatively easy to label. Nadal is the fighter, the supreme in-the-moment competitor. Federer is the pleaser, preternaturally elegant on the move or at rest. But Djokovic, less beloved on a global scale than his rivals, has been harder to pin down. Perhaps his description should spring from that elusiveness. Call him the searcher.

He is a searcher, said Marian Vajda, his longtime coach and confidant. It seems that the things are perfect, but suddenly he wants to change in some way.

Through the years, Djokovic has switched to a gluten-free and dairy-free diet, practiced meditation and visualization and tinkered repeatedly and not always successfully with his service motion. He has used a personal hyperbaric chamber during tournaments. Most recently, he split with an analytics consultant in part because Goran Ivanisevic, one of Djokovics coaches, believed they needed get to back to more basics and not rely on all these numbers.

But Djokovic is committed to experimentation.

There are so many athletes, so many tennis players, who play so well in practice, and then it comes to the match, and its a different story, he said. You might have a good match or two or a good month or two, but how can you consistently be there? That tonic or formula of success is like a holy grail for any athlete. How can I really optimize everything and be in a balanced state of mind, body and soul every season for the rest of my career and really be able to peak when I need to?

I think the No. 1 requirement is constant desire and open-mindedness to master and improve and evolve yourself in every aspect. I know Roger has been talking about it, and its something I feel most top athletes of all sports agree on. Stagnation is regression.

For Vajda and for Djokovic, the restlessness helps explain the pull of the philosophical. Djokovic is trying to channel some of his innate fire and temper it, too.

There were the years when he was very impatient, Vajda said. All the time, he was saying, When, when, when am I going to be No. 1? And I would say, When it arrives, just be patient but we have to do certain things in a proper way in order to get there.

Djokovic can be a challenging pupil and a challenging personality. On the court, there are still times when he seems most effectively and evidently fueled by anger.

But there is also an emerging side, reflected in his marathon victory over Federer in last years Wimbledon final, when he saved two match points and prevailed in a five-set duel that for all its brilliant points of light and fight, felt above all like a battle of concentration with neither player wanting to break out of his tennis-sensei bubble to give the other any emotional fuel.

It was a strikingly different vibe from that of another epic final: Djokovics 2012 Australian Open victory over Nadal, which was full of fiery fist pumps, bulging eyes and grinding rallies that, after nearly six hours, left neither man able to stand at the awards ceremony.

The Australian Open was an out-of-body experience, Djokovic said. But in Wimbledon I was more conscious of what was happening to me.

It has been just about an hour since the interview began, and there is a knock on the door. Djokovic gets up as the door opens to reveal a slightly stooped figure wearing a sweatshirt with the hood up: Agassi.

He is in Monte Carlo, advising another player, Grigor Dimitrov, and they have the conference room booked for a team meeting.

Agassi and Djokovic do a double take.

Do you believe in coincidences? Djokovic says in a booming voice.

Never, never, never, Agassi says.

They exchange an embrace and news, including an update on Agassis son with his wife, Steffi Graf Jaden, a talented pitcher who has committed to play baseball at the University of Southern California.

The Yankees were in the house the other day, so the draft is always possible, Agassi says. Its scary, different than tennis in that you dont eat what you kill. The next level has to believe in you to get a chance.

Agassi makes it clear he still believes in Djokovic.

Lifes not done, and I understand everybody is still playing, Agassi says of the Big Three. But hell be recognized as the best.

Djokovic says it helps to have a clear purpose, even if it is a different purpose.

I had to find my reason, Agassi says. Its so important to have that reason.

Agassi soon excuses himself, and Djokovic begins speaking about parenthood. He knows he will not be slamming a 10-euro note on his kitchen table. He wants his children to know about his childhood but not to be burdened by it.

I definitely dont want my kids to go, Oh my God. Here he goes again, saying I didnt have this, I never had that, and you have everything, he says. I dont want that because my kids are born in this family in this way and these circumstances, and I respect that.

There is soon another knock on the door. This time it is Dimitrov. They need the room, and Djokovic needs to pick up Stefan at school.

Down the stairs, into the street and past the Lamborghini, Djokovic breaks into a run, dodging traffic with a backpack slung over one shoulder.
